Coordination of Care – Create a Form

Last week I had to take Mom to one of her psychiatry appointments.  Typically, the appointments should only take 10-15 minutes.  Hers usually last the better part of 40-60 minutes.

After the appointment I had to meet with the residential nursing staff at Mom’s facility, update them on what the physician changed, the tests he ordered, and then get feedback from them.  Then, I’ve got to communicate all of the nursing staff’s needs back to the doctor.  By the time it was all said and done, it was a total of six hours before I was able to move-on to my next task of the day.

Attached is a form document that I’m going to start using.  There is a lot in there that shortcuts things, e.g. having contact information, so the providers can get a hold of one another, making a bold action item area for each office, then providing a short narrative of what was going on so everyone has context.

It worked.  After typing this up, I faxed it to both providers, and received calls from both it was received and they completed their part.

I’ve attached a copy of my notes in the link below.  The bottom portion was literally a copy and paste from my note app.  Then I kind of do a little decompress handwritten portion once everything was done.  Reflect a minute – think about what I heard – and try to figure out what changes I need to make based upon what I heard from everyone.
